New Student Registration - Documents Required

At the time of registration, you must present the following original documents:
Proof of Residency
One document is required from each group listed below. (All documents, except the passport, must show your new District 68 address.)
A. For Renters: Original current signed lease listing all occupants of the unit (not a photocopy) For Homeowners: Original mortgage coupon booklet or statement, or current tax bill with taxpayer name and address indicated on the tax bill. New homeowners may present their Warranty Deed. (no photocopies)
B. Provide at least two (2) of the following original documents. The address must match the document presented in part A above.
Parent/Guardian Identification (to notarize your signature):
Proof of Age -- Child MUST be 5 years old by September 1, 2011
The exact dates of all immunizations must be supplied. Rubeola (measles)
Poliomyelitis Rubella (German measles)
Hepatitis B Mumps Lead Screening (age 6 & under)
Varicella (chickenpox): either date of immunization or date disease was contracted
Tetanus/Pertussis (whooping cough)/ Diptheria
Please note: If your child received the measles vaccine before the age of 12 months, she/he should have two additional vaccinations. You physician will advise if re-immunization is necessary.

- Illinois Driver's License
- Current Electric Bill
- Illinois State I.D.
- Homeowner's or Renter's Insurance Statement
- Current Skokie Water Bill
- Village/Town Vehicle Sticker Receipt
- Current Natural Gas Bill

- For example: Valid Drivers License or State I.D., valid passport, valid consulate identification card, work identification card, etc.
Proof of Age -- Child MUST be 5 years old by September 1, 2011
- Child's Original Certified Birth Certificate (If the birth certificate is in a language other than English, it must be translated into English by an accredited translation service.). Hospital-issued certificates are not valid birth certificates.
- If a birth certificate is not available because the child has recently moved to the U.S., the child's valid passport will be accepted.
- For students transferring from another Illinois public school: this form is issued by the student's previous school
Physical, Dental and Vision Examination
- State law requires that all students have the following examinations:
- A complete physical examination for students entering kindergarten and 6th grade or new to U.S. Students transferring from another U.S. school may not need a new physical. The health form from the previous school will be examined for State of Illinois compliance.
- A complete dental examination for students entering kindergarten, 2nd and 6th grades
- A complete vision examination for students entering kindergarten and all students attending an Illinois school for the first time
- Examination forms must be completed before a child is admitted to school. Transfer students must have a completed health and immunization form returned to the principal's office within 30 days after enrollment as a condition for continued attendance.

Student Fees
- Fees will be collected at the time of registration. Students registered for 2011-12 prior to July 2011 will receive a fee bill in the mail. We accept cash, check, Visa, MasterCard, or Discover. Information on need-based financial assistance will be available at registration.
Required Fees - Material Fee - all grades $ 115.00 per student
Optional Fees - Bus Fee $ 235.00 per student
(No fee is charged if the residence is more than 1.5 miles from school. Mileage is determined by our bus routing software.) - Yearbook Fee - Gr. 6-8 $ 15.00
- Parent/Teacher Association Dues $ 10.00 per family per school
