Parental Authorization for Student's Network and
Information Services Access
I hereby certify that I have read the attached policy and regulations regarding students' use of the District's computer network and outside information services including, but not limited to, the Internet.
I understand that the network, Internet and information services allow an individual virtually unrestricted access to individuals and information contained in computers throughout the world. I further understand that they are a valuable educational resource, and that the School District intends to instruct students in and to have them utilize this resource in a manner that is consistent with the District's educational program.
I understand that, although the policy and rules and regulations are intended to restrict students' use to acceptable educational uses, it is possible that students may access inappropriate materials or may use the network and services in an illegal manner resulting in purchases or financial obligations for which we, as parent(s) or guardian(s) agree to be liable.
I understand that although the Board Policy and Rules and Regulations restrict my student's use to acceptable educational uses, the School District may not be able to prevent students from using the Internet in a manner that is prohibited by the Policy and its Rules and Regulations. Therefore, I understand that the District cannot guarantee compliance with the District's acceptable use policy. I further understand that prior to signing this authorization I may discuss the District's Internet access policy with appropriate school personnel.
Based upon these understandings, I hereby request that my child receive the necessary account number and password to access the District 68 local area network including the Internet and outside information services. I agree to hold harmless and indemnify Skokie School District 68, its employees, agents and Board members for any harm and unauthorized costs caused by materials, transactions or software obtained through such computer use. I accept full responsibility for supervision of my child's use of computer information services in other than a school setting and agree to prohibit unauthorized access to the District's computer network from other than a school computer.
