Teacher and Non-Student Authorization
for Internet Access
I, the undersigned, agree that I have read the attached Rules and Regulations regarding the use of the District's Internet account and agree to abide by its terms and conditions. I understand that any unacceptable use shall be grounds for the denial, revocation, or suspension of my Internet privilege; may result in additional discipline up to and including dismissal; and may result in criminal or civil penalties. I understand that the District makes no warranties of any kind, whether express or implied, regarding the Internet. In addition, I agree to indemnify the District for any losses, costs, or damages, including reasonable attorney fees, incurred by the District relating to, or arising out of, any violation of these Rules and Regulations and any unauthorized charges or fees, including telephone charges, long-distance charges, per-minute surcharges, and/or equipment or line costs. In consideration for the District's Internet account, I hereby release Skokie School District 68 and its Board members, employees, and agents from any claims and damages arising from my use, or inability to use the Internet.
