School-based Flu Vaccination Clinic
The Village of Skokie has partnered with Advocate Children’s Hospital to operate school-based flu vaccination clinics for students currently enrolled in Skokie School District 68.
Participation in the flu vaccination clinic is voluntary. Participating students will be given the injectable vaccine. If you would like for your student to receive the vaccine at school, please fill out the Influenza Vaccine Consent Form sent home by your child's school making sure that all relevant insurance information is included. Your form must be completely filled out and returned to your child’s school no later than Wednesday, October 31, 2018. If you have any questions about your child receiving this vaccine, please contact your child’s primary care physician. Consent forms will NOT be accepted after Wednesday, October 31, 2018. If the response rate exceeds the vaccines on hand, a second day will be scheduled and you will be notified.
